Tirzepatide (Dual GIP and GLP-1 Receptor Agonist): Zepbound activates two of your body’s metabolic hormone receptors at the same time, GLP-1 and GIP. GLP-1 reduces appetite and slows stomach emptying. GIP enhances insulin sensitivity and supports fat metabolism. Hitting both pathways together produces larger weight loss on average than activating GLP-1 alone. Your dose is increased gradually over several months.
Best for: Adults with a BMI of 30 or higher, or 27 or higher with a weight-related condition, who want the most aggressive FDA-approved pharmacologic option for sustained weight loss.
Not recommended for: Anyone with a personal or family history of medullary thyroid carcinoma or MEN 2 syndrome, those with a history of pancreatitis, anyone pregnant or breastfeeding, and anyone already taking another GLP-1 medication.
Most common side effects are nausea, diarrhea, vomiting, constipation, and stomach pain, especially during the first weeks and dose increases. These usually improve as your body adjusts. Rare but serious risks include pancreatitis, gallbladder disease, kidney problems, and (based on animal data) a theoretical risk of medullary thyroid tumors. Zepbound carries a boxed warning for medullary thyroid carcinoma risk.
