Tadalafil (Sublingual/Buccal Delivery): Tadalafil acts the same way regardless of delivery route: it inhibits PDE5, increases blood flow to the penis, and lets erections happen in response to sexual stimulation. The troche format absorbs across the oral mucosa, partially bypassing first-pass liver metabolism. Effects can last up to 36 hours similar to the pill.
Best for: Adult men with erectile dysfunction who want the convenience of a no-water dosing format, or who would prefer faster onset than the pill provides.
Not recommended for: Anyone taking nitrates or certain alpha-blockers, those with significant cardiovascular disease, severe liver or kidney disease, recent stroke, or a history of priapism.
Most common side effects are similar to the pill: headache, flushing, nasal congestion, and back pain. Mild local mouth irritation can also occur. Rare but more serious: priapism, significant blood pressure changes, and the cardiovascular risks of combining tadalafil with nitrates.
