Mounjaro (Tirzepatide): FDA-Approved Weekly Injection for Type 2 Diabetes

Mounjaro is an FDA-approved injectable containing tirzepatide, taken once weekly for adults with type 2 diabetes. It activates two metabolic hormone pathways at once, GLP-1 and GIP. While it isn’t FDA-approved specifically for weight loss, it produces clinically meaningful weight reduction alongside HbA1c control, and providers prescribe it for that purpose when clinically appropriate. The same molecule is sold as Zepbound under the weight-loss indication.

Tirzepatide (Dual GIP and GLP-1 Receptor Agonist): Mounjaro activates both GLP-1 and GIP receptors. GLP-1 lowers appetite, slows stomach emptying, and stimulates insulin release in response to meals. GIP enhances insulin sensitivity. Together, the two pathways produce stronger blood sugar control and more weight loss than GLP-1 alone in head-to-head studies.
Best for: Adults with type 2 diabetes who want strong glycemic control alongside meaningful weight loss. Particularly well-suited for people whose HbA1c has been difficult to control on other medications.
Not recommended for: Anyone with a personal or family history of medullary thyroid carcinoma or MEN 2 syndrome, those with a history of pancreatitis, anyone pregnant or breastfeeding, and anyone already taking another GLP-1 medication.
Most common side effects are nausea, diarrhea, vomiting, constipation, and stomach pain, particularly during the first weeks and dose increases. These usually improve as your body adjusts. Rare but serious risks include pancreatitis, gallbladder disease, kidney problems, hypoglycemia (especially when combined with insulin or sulfonylureas), and (based on animal data) a theoretical risk of medullary thyroid tumors. Mounjaro carries a boxed warning for medullary thyroid carcinoma risk.
