Minoxidil 7% + Tretinoin + Fluocinolone + Finasteride Spray: Topical Combination Targeting DHT and Growth

A high-strength topical that combines minoxidil’s vasodilation with topical finasteride’s DHT reduction, plus follicular support.

This compounded topical spray combines high-strength minoxidil (7%) with topical finasteride. Topical finasteride is increasingly used as an alternative or supplement to oral finasteride; applying it locally to the scalp can reduce scalp DHT with much lower systemic exposure than the oral pill. Tretinoin enhances minoxidil absorption and fluocinolone reduces follicular inflammation. The combination is designed for patients who want comprehensive topical therapy without (or with reduced) systemic finasteride exposure.

Minoxidil 7%: Minoxidil is a topical vasodilator that’s been used for hair loss for decades. It improves blood flow to follicles, extends the active growth phase of the hair cycle, and can stimulate dormant follicles. It works for both men and women and is typically applied once or twice daily to areas of thinning or loss.

Tretinoin (Topical Retinoid): Tretinoin enhances absorption of topical minoxidil and increases skin cell turnover.

Fluocinolone (Topical Corticosteroid): Fluocinolone reduces follicular inflammation.

Topical Finasteride: Finasteride blocks the enzyme 5-alpha-reductase, which converts testosterone into DHT. DHT is the hormone primarily responsible for shrinking hair follicles in pattern hair loss. Lowering DHT lets follicles recover, slowing or stopping further loss and (for many men) supporting regrowth. Applied topically, finasteride can reduce scalp DHT with significantly less systemic absorption than the oral pill, which may translate into fewer sexual side effects for many patients.

Best for: Adult men with pattern hair loss who want both vascular and hormonal support topically, and who would prefer to minimize systemic finasteride exposure. Particularly well-suited for patients who haven’t tolerated oral finasteride.

Not recommended for: Pregnant or breastfeeding individuals (or anyone who may become pregnant), people with active scalp infections, and those allergic to any component. Topical finasteride still carries some absorption risk; patients trying to father a child should discuss with their provider.

Most common side effects are scalp irritation, redness, dryness, and itching. An initial shed is normal in the first 4 to 8 weeks. Topical finasteride may still cause some of the same side effects as oral finasteride (changes in libido, erectile function) in a smaller proportion of patients due to limited systemic absorption.

What to know before you start.

Minoxidil 7% / Tretinoin / Fluocinolone / Finasteride Spray is a compounded medication prepared by a licensed pharmacy for individual patient use.

It has not been reviewed by the FDA for safety or efficacy, and is prescribed based on the clinical judgment of a qualified healthcare provider. Compounded drugs are permitted to be prescribed but are not FDA-approved for safety and efficacy.

Clinical Evidence

Topical finasteride has been shown to reduce scalp DHT with less systemic absorption than oral finasteride in pharmacokinetic studies, and has demonstrated efficacy in pattern hair loss in head-to-head trials with oral. Combined with minoxidil, the dual-mechanism approach is supported by multiple clinical studies. (Source: Caserini et al., Journal of Dermatological Treatment, 2016)
