Finasteride is an FDA-approved oral medication taken once daily to slow or stop male pattern hair loss. It works by reducing levels of dihydrotestosterone (DHT), the hormone that causes hair follicles to shrink and stop producing visible hair. Most patients see hair loss slow within 3 to 6 months and, in many cases, some regrowth over 6 to 12 months. The same active ingredient is sold under the brand name Propecia.

Finasteride (5-Alpha-Reductase Inhibitor): Finasteride blocks the enzyme 5-alpha-reductase, which converts testosterone into DHT. DHT is the hormone primarily responsible for shrinking hair follicles in pattern hair loss. Lowering DHT lets follicles recover, slowing or stopping further loss and (for many men) supporting regrowth.
Best for: Adult men with male pattern hair loss who want to address the hormonal driver of thinning. Particularly effective when started earlier in the process before significant miniaturization has occurred.
Not recommended for: Anyone pregnant or who may become pregnant should not handle crushed or broken tablets (DHT reduction can affect fetal development). Men actively trying to father a child should discuss with their provider given potential effects on semen parameters. Patients with significant liver disease should also discuss carefully.
Most common side effects are mild and uncommon overall (under 5% of users): reduced libido, erectile difficulty, and reduced ejaculate volume. These typically resolve when finasteride is stopped, though a small minority of patients have reported persistent symptoms after discontinuation (post-finasteride syndrome, an area of ongoing research). Less common considerations include changes in mood, breast tenderness or enlargement, and (rarely) more serious effects requiring discontinuation.
